COMMUNITY REINTEGRATION PROGRAM

Transitioning from hospital to home with confidence and support.

At the Split Second Foundation, we understand that returning home after a hospital stay can be a significant transition for individuals and their families. Our Community Reintegration Program is specifically designed to support families during this critical phase, ensuring they have the resources and support needed to reintegrate into the community smoothly and successfully.

Durable Medical Equipment (DME):

Ensuring our clients have access to the necessary durable medical equipment is paramount. We help coordinate the acquisition of essential items such as wheelchairs, walkers, hospital beds, and more to ensure the home environment is fully equipped to support the individual’s physical needs.

Home Modification Assessments:

Safety and accessibility in the home are crucial for a successful transition. Our team provides comprehensive assessments to identify potential modifications that can make homes more accessible. This may include installing ramps, modifying bathrooms for easier access, or adjusting living spaces to better accommodate mobility aids.

Transportation Services:

Mobility doesn’t end at the front door. We assist with arranging transportation solutions for appointments, community engagement, and day-to-day errands. Our goal is to help individuals maintain their independence and stay connected with their community.

Direct Care Services:

For families that require additional support, we offer connections to direct care services. This can include nursing care, physical therapy, or daily living assistance, depending on the specific needs of the individual and their family.

Holistic Support:

Our approach goes beyond physical needs, addressing the overall well-being of our clients and their families. We provide resources and support to help manage the emotional and psychological challenges that can accompany this transition.
